- W2912588096 abstract "Introduction: Diverticular bleeding accounts for 17 to 40% of lower Gastrointestinal bleeding. our aim is to quantify and compare the trends of hospitalizations for Diverticular bleeding and diverticulitis by sex and race. we also compared the trends of surgical interventions and mortality. Methods: We analyzed data from the Nationwide Inpatient Sample from 2005 to 2014. We identified adult patients with a discharge diagnosis of diverticular bleeding or diverticulitis, we calculated age, sex, and race-specific rates. Results: The prevalence of combined Diverticular and Diverticulitis bleeding(CDV), Diverticular bleed(DV) and Diverticulitis bleed (DiV) decreased over the ten year period. CDV decreased from 391/100,000 in 2005 to 300/100000 in 2014 (23.2% drop). CDV prevalence rate was 308/100,000 admission.The prevalence of Div decreased more than the DV 31.7% vs 27.7.The DV bleed is more common in males where as the DiV bleed is more common in females.Div patients tend to younger than the DV bleed, mean age 72.3 vs 76.3.female patients in bothe groups older than the male counterparts.DV bleed is more common in Balck race whereas the DiV bleed is more common in the white race. Mean length of stay is slightly longer for DiV patients vs DV, 5.76 vs 5.13, P < 0.05. The prevalence of CDV increases with advanced age, almost 90% of the admissions and 95% of in-hospital deaths are from the age group>60. The mortality rate is been steadily decreasing in both the groups. In-hospital mortality is high for the Div group compared the DV group (OR of 1.33). mortality rate was 1.9%.on Logistic regression, Surgery and age above 60 are associated with mortality, Race has no effect on mortality. The surgical intervention rates are trending down,5% of the patients will need surgery. The surgical intervention is more often seen in the below 60 age group, 7% vs 4.7%(OR 1.59, 95% CI 1.44 to 1.65, P < 0.001). Div patients will need surgery more often than the DV,9.8% vs 4.2%(OR:2.5, 95% CI 2.4 to 2.6, P < 0.001) with no significant difference in the surgical mortality. Surgical patients are with higher mortality with the OR of 6.35. Surgical mortality is fluctuating over the ten year period with the mortality rate of 8.5%.FigureFigureFigureConclusion: The hospitalization for Diverticular bleed decreased for first few yeas, then plateaued. The age and race specific hospitalization rates are different for Div and DV bleeding indicating different mechanisms of pathogenesis, However, race has no effect on mortality." @default.
