- W2912758634 abstract "Introduction: Constipation-predominant Irritable Bowel Syndrome (IBS-C) is a common disorder with a reported prevalence of 16% in N America. Alteration of the gut microbiome may result in altered GI motility observed in patients with IBS-C, playing a role in the disease pathogenesis. Eradication of bacterial overgrowth causing dysbiosis can result in some normalization of motility, while the use of fecal microbiota transplantation (FMT) can provide an effective and low-risk therapy. The orally administered encapsulated lyophilized FMT product is preferable to colonoscopically or enema-infused therapy. Here, we report the novel use of encapsulated lyophilized full-spectrum microbiota (LFSM) in treating a patient with severe IBS-C accompanied by chronic fatigue syndrome. Case Report: A 26 y/o female with a 6 y history of GI symptoms presented with bowel opening every 3-5 days, severe straining, difficulty passing a motion, continuous abdominal pain of moderate severity and bloating (Table 1). Furthermore, the patient complained of chronic fatigue, inability to concentrate with marked “brain fog”. She was bed-ridden, unable to continue work for the last 6 months, or travel. Collection of stool for LFSM production was carried out as per AGA Consensus Guidelines. Stool was homogenized with cryoprotectant and the resulting slurry was lyophilized and encapsulated in DR gel capsules containing ˜1.6 x 1011 viable cells/capsule. The patient was treated with a total of 138 capsules over a 12-week induction period. During this period, symptom questionnaires were collected and stools cultured for pathogens to assess efficacy and safety of the LFSM treatment. Discussion of Findings: The patient noted significant improvement in symptoms commencing at week 2. By week 12, numerous symptoms including abdominal discomfort/pain improved and chronic fatigue symptoms ceased. There was a marked increase in energy levels and appetite, resulting in successful completion of half marathons. She is able to travel, recommenced work, and no longer notices “brain fog”. Overall, her QoL improved dramatically by post-treatment and is currently continuing on maintenance. The LFSM treatment appears to have normalized the gut microbiome and the therapy has shown to be efficacious and safe for the last 16 weeks of treatment.
