- W2912940085 abstract "Gastric ulcers are most commonly associated with nonsteroidal anti-inflammatory drugs (NSAIDs), Helicobacter pylori (H.pylori) infection, or other common medications such as steroids, anticoagulants, selective serotonin reuptake inhibitors, and bisphosphonates. Deferasirox is an oral iron chelator used to reduce iron overload with the most commonly associated side effects including renal dysfunction, skin rash, diarrhea, vomiting, and abdominal pain. In the pediatric population, single cases of a gastric ulcer and perforated duodenal ulcer have each been reported. However, ulcer disease has not been reported in the adult population. A 74-year-old woman with hemochromatosis presented to the emergency department with dizziness and was found to have a microcytic anemia with Hgb 9.0 gm/dL and Hct 25.4% down from her baseline of 11 gm/L and 32%, respectively. Rectal exam revealed brown stool positive for fecal occult blood. Her hemochromatosis was being treated with a combination of phlebotomy and recently started on deferasirox (Exjade). She had been phlebotomized one week prior to admission, and had inadvertently taken four times the recommended dose of deferasirox at that time given it was recently prescribed and she was unsure of dose. She denies any other medications. Despite a lack of overt gastrointestinal bleeding her hemoglobin kept decreasing and she required multiple blood transfusions. Therefore, and upper endoscopy and colonoscopy were done for further evaluation. The upper endoscopy showed a normal appearing esophagus with multiple 3-4 mm white-based ulcerations throughout the gastric body, and a large 1 cm white-based antral ulcer. Multiple erosions were seen in the duodenal bulb and a 1 cm white-based ulcer was present in the duodenal sweep as well. Biopsies were negative for H. pylori. The colonoscopy did not show a source of bleeding. She was started on a proton pump inhibitor and did not require any further transfusions. We present a patient with hemochromatosis who developed gastric and duodenal ulcers likely from deferasirox use. Deferasirox is not commonly known to cause peptic ulcers. In our current knowledge, this is the first case-report on this adverse drug reaction in the adult population. Physicians should be aware of the possibility of deferasirox causing ulcer disease. Consideration for concomitant H2 blocker or proton pump inhibitor with deferasirox may be given to prevent this complication." @default.
