We clearly agree with his conclusion that r-hCG is as good as or even better than u-hCG. We do not agree, however, with his criticism of the design of the prospective, randomized trials included in our paper. Dr. Butler is extrapolating molar concentrations for u-hCG and r-hCG. This cannot be done. It is also not clear what the origin is of the data from which he derived his figures. He based the rationale for doing this on the publication of Birken et al. (2Birken S. Berger P. Bidart J.M. Weber M. Bristow A. Norman R. et al.Preparation and characterization of new WHO reference reagents for human chorionic gonadotropin and metabolites.Clin Chem. 2003; 49: 144-154Crossref PubMed Scopus (85) Google Scholar), who advocate that calibrate reference preparations should be expressed as moles/liter. This recommendation, however, is only intended for measurements by immunoassays, and it is not valid for therapeutic preparations for which potency is measured by bioassays. Bioassays reflect activity because the results depend on quantity, the overall profile of degree of glycosylation (isoforms), and the half-life of the product. As we stated in our paper, the potency of 250 μg r-hCG is comparable to approximately 6,500 IU u-hCG—not twice as high compared with 5,000 IU. We find no proof in the literature for this suggestion. The design of the reviewed prospective, randomized studies was therefore correctly chosen." @default.
