Depression is an increasingly prevalent, and often disabling, chronic condition with large impacts on public health, and the costs to both individuals and the society are substantial. Methods:* We conducted a prospective cohort study among 50,306 participants in the Nurses’ Health Study (mean age: 67 years) without depression at baseline (1996). These nurses lived across the contiguous U.S. and were followed until 2008 via biannual questionnaires. We examined the association of onset of depression with particulate matter with diameter <2.5µm (PM2.5) estimated at their residence. We defined onset of depression as the first report of clinical diagnosis or antidepressant use. Exposures were assessed as the 1-, 2- and 5-yr averages prior to the 2-year cycle during which depression status was assessed. We also conducted sensitivity analyses using separately reported diagnosis or antidepressant use as the defined onset. We employed Cox models, adjusting for individual and census-level characteristics. Results:* We observed 7145 incident cases of depression among study participants. We found positive associations between PM2.5 exposures and onset of depression that were stronger for shorter durations (HR:1.07, 95%CI=0.98-1.17 and 1.04, 95%CI=0.95-1.14 per 10 µg/m3 increase in 1 and 5-yr PM2.5, respectively). Associations with clinical diagnosis only (6.9%) were null (1-yr PM2.5 HR:0.96, 95%CI=0.85-1.10), whereas associations with antidepressant use (12.8%) were stronger (1-yr PM2.5 HR:1.10, 95%CI=1.00-1.20).Conclusions:* To our knowledge, this is the first study to examine the association between long-term PM2.5 exposures and onset of depression in a cohort across the United States. PM2.5 exposure was associated with incident depression when using antidepressant use, but not clinical diagnosis, as the outcome." @default.
