- W2913899936 abstract "Introduction: Gender and racial inequality in healthcare quality has been extensively documented. Furthermore,high variation in the care received by inflammatory bowel diseases (IBD) patients has also been reported. We aimed to assess potential gender or racial disparities in adherence to established IBD quality of care indicators (QIs) by gastroenterologists practicing in the community and in a specialized IBD practice at an academic medical center. Methods: A prospective review of consecutive outpatient visits at the gastroenterology clinic at a tertiary academic medical center was conducted over 4 months (January-April 2017). Demographic information and QIs developed by the AGA and CCFA including documentation of IBD phenotype, vaccination status, corticosteroid usage, and appropriate endoscopic evaluation were recorded. We analyzed for gender and racial (Caucasian vs. African American) differences in QIs using t-test and Chi square test. (Prism, Irving CA). Results: Over the study period, 478 patients (median age 42 years, 51% female, 14% African American) were evaluated in the IBD clinics. Disease duration was a median of 14 years, and 55% had Crohn's disease. There was no significant gender (male vs. females) differences in QIs: IBD phenotype documentation (67% vs. 68%), HBV vaccination (74% vs. 69%), Influenza vaccination (54% vs. 50%), Pneumovax vaccination (31% vs. 35), and steroid use greater than 12 weeks (25% vs. 28%), colorectal cancer surveillance (58% vs. 64%), endoscopy within 6 months of modifying therapy (45% vs. 39%). There were no significant racial (Caucasian vs. African American) differences in adherence to most QIs: IBD phenotype documentation (71% vs 67%), HBV vaccination (71% vs. 76%), Influenza vaccination (50% vs. 66%, p=0.01), Pneumovax vaccination(32% vs 42%, p=0.08), steroid use greater than 12 weeks (26% vs 28%), colorectal cancer surveillance (65% vs. 60%), and endoscopy within 6 months of modifying therapy (46% vs. 37%,). In addition, race and gender did not contribute to significant differences in QIs in subgroups of patients managed either by community general gastroenterologists vs IBD specialists at an academic medical center. Conclusion: There was no consistent evidence of gender or racial disparities in adherence to most of the quality of care indicators in IBD either in community or specialized IBD practices. This requires confirmation in future studies replicated at other community and academic IBD practices." @default.
