- W2914462851 abstract "Esophageal diverticulae are classified according to their location: proximal (pharyngoesophageal or Zenker), middle, and distal (epiphrenic). Middle and distal diverticulae are subdivided as pulsion and traction types. Traction types arise from chronic mediastinal diseases causing scar formation. Pulsion diverticula occur with a distal esophageal motility disorder, forming an epiphrenic diverticulum. The majority of patients are asymptomatic, or may present with severe dysphagia, gastroesophageal reflux disease, weight loss, cardiac arrthythmias, halitosis, or recurrent episodes of aspiration pneumonia. We present a case of epiphrenic diverticulum that highlights the importance of high clinical suspicion and the difficulty diagnosing this disease. A 64-year-old man with a history of 10-pound weight loss in previous 2 months was referred for EGD for further work up for dysphagia, after chest CT found middle and lower esophageal wall thickening and distension with air and fluid at the level of the lower third of the esophagus. EGD revealed moderately dilated esophagus with stasis esophagitis in the entire esophagus. A diverticulum with a large opening and no stigmata of recent bleeding was found in the lower third of the esophagus several cm proximal to the lower esophageal sphincter (LES) area. In addition, a hypertonic LES was found. There was moderate resistance to endoscope advancement into the stomach. The Z-line was regular. The gastroesophageal junction and cardia were normal on retroflexed view. A through-the-scope (TTS) dilator was passed through the scope. Dilation with a 15-16.5-18 mm x 5.5 cm CRE balloon (to a maximum balloon size of 18 mm) dilator was performed. Motility study showed occasional simultaneous contraction, hypertensive LES with incomplete relaxation, suggestive of evolving type 3 achalasia. We prescribed fluconazole for esophagitis, and preprandial sublingual nitroglycerin as needed for LES relaxation. Two months following endoscopy the patient reports improved dysphagia and weight gain. Epiphrenic diverticulum continues to be a rare diagnosis despite improvements in diagnostic studies such as manometry and imaging. Diverticulae can present with variety of symptoms that can mimic more common diseases, or may be asymptomatic in as many as 40% of patients. Undiagnosed, epiphrenic diverticulae can lead to or further worsen regurgitation, gastrointestinal bleeding, aspiration, and cancer." @default.
