- W2914911207 abstract "Extrapulmonary squamous cell carcinoma (SCC) is a rare condition, accounting for 0.1-1% of all gastrointestinal (GI) tumors. In the GI tract, these are most commonly found in the esophagus (0.6-2.8%). These tumors are usually quite aggressive with presence of metastasis at the time of diagnosis, ultimately leading to a poor prognosis. We present to you one such case. Case. 76-year old male with history of diverticulosis, internal hemorrhoids, prostrate cancer status post seed implant presented with complaints of difficulty swallowing, both solids and liquid associated with pain since 1 month. No history of weight loss/loss of appetite was present. He underwent esophagogram which showed mucosal abnormalities in the mid-section of esophagus suspicious for malignancy. Esophagoduodenoscopy (EGD) was done which showed presence of an irregular mass at 30 cms. Biopsy showed tumor cells with nuclear molding, brisk mitotic rate, karyorrhexis, positive for CAM 5.2, CD56, synaptopysin and weakly positive for chromogranin suggestive of SCC. PSCCE was first described by McKeown in 1952. The clinical presentation of this tumor is similar to other types of esophageal malignancies with most common symptoms being dysphagia, weight loss, loss of appetite, reflux like symptoms and chest pain. It is usually located in the lower 2/3rd of the esophagus. Biopsy is required for diagnosis with histological criteria being the same as defined by WHO for pulmonary SCC. Various immunohistochemistry markers have been found to be associated with SCC such as CD56, synaptopysin, enolase and chromogranin. Protocols for treatment are based on clinical experience and not based on prospective studies due to the rarity of the condition. Limited disease, defined as disease confined to the esophagus and surrounding organs with or without lymph node involvement, is usually treated with surgery and chemo-radiation.
