- W2914973793 abstract "Introduction: Ampullary adenomas are a type of benign neoplasm of the ampulla of Vater that may occur sporadically or in the setting of familial polyposis syndrome. In the absence of familial adenomatous polyposis, they are usually found in patients over the age of forty. Case Presentation: A 25 year old female with no significant past medical history initially presented to her primary care physician complaining of three to four weeks of intermittent right upper quadrant pain. Initial work up revealed abnormal liver tests in a cholestatic pattern. A right upper quadrant ultrasound was also performed and revealed a dilated common bile duct of 1 cm in the absence of cholelithiasis or choledocholithiasis. Upon evaluation, the patient denied fever, jaundice, or pruritus. Examination revealed mild right upper quadrant tenderness to palpation without rebound or guarding. There was no evidence of scleral icterus or jaundice. Repeat laboratory examination showed a normal hepatic function panel. MRCP showed a distal common bile duct lesion with radiographic features suggestive of a polyp/mass. The patient underwent endoscopic ultrasound that revealed a bulging ampulla with an intramural lesion. Subsequent ERCP revealed a large villous mass once a sphincterotomy was performed. Pathology revealed fragments of adenoma, but did not reveal evidence of carcinoma. The patient was referred for and underwent successful endoscopic ampullectomy with adjunctive placement of biliary and pancreatic stents. Post-procedure, the patient did well and is scheduled for post-treatment surveillance. Discussion: Sporadic ampullary adenomas are usually diagnosed in patients over the age of forty, and most commonly in patients above the age of seventy. The most common presenting symptom is epigastric/right upper quadrant pain or the development jaundice from biliary obstruction. The development of cholangitis is rare. Given the malignant potential of these lesions, treatment strategies include resection versus endoscopic surveillance. Resection techniques include pancreaticoduodenectomy, surgical ampullectomy, or endoscopic ampullectomy. Given the size of the lesion and absence of lymph node involvement, the decision was made to pursue endoscopic ampullectomy, which was successful.Figure: Ampullary Mass." @default.
