• W2917103478 abstract "Circulating cell-free DNA (cfDNA) in peripheral blood provides a “liquid biopsy” that can be used to non-invasively profile tumor genetics for diagnosis and treatment guidance. This promising approach is currently limited by the low quantity of cfDNA routinely present in the bloodstream. The goal of this study was to test the hypothesis that transcatheter arterial embolization (TAE) for hepatocellular carcinoma (HCC) leads to transiently increased spillage of tumor DNA, thus increasing cfDNA yield and allowing for improved detection of tumor mutations. Blood samples were obtained preprocedure, 1 day after, and 2 weeks after TAE in 21 patients (19 males; mean age=59) with HCC (mean tumors/patient=1.5; mean mRECIST size=4.6 cm). cfDNA was isolated and quantified, and concentrations before and after TAE were compared using paired T-tests. As proof-of-concept whole exome sequencing of cfDNA was performed to a depth of 500 reads per base, and sequence data was compared to normal germline DNA to identify tumor mutations in 2 patients. cfDNA was successfully isolated in all patients. At baseline prior to TAE the mean cfDNA concentration was 0.55 ng/uL. The mean concentration was significantly increased on the day following TAE (0.89 ng/uL; p=0.004) and returned to baseline levels by week 2 (0.67 ng/uL; p=0.23). Whole exome sequencing performed on cfDNA obtained the day after TAE in 2 patients yielded 18,276,169,636 and 14,578,527,842 base pairs with 92.22% and 91.64% high quality reads, respectively. There were 65 and 72 mutations in these 2 patients, including potentially impactful mutations in the oncogenes/tumor suppressors TCHH, ANTX, COL4A4, MCF2L, ARID3B, NCOA3, IL3RA, PCDHB14, and OR7A17. TAE induced a transient but significant increase in cfDNA yield, allowing for whole exome sequencing and identification of tumor mutations using conventional techniques. The sharp increase in cfDNA following embolization therapy may be a tool to improve “liquid biopsy” of HCC and promote further study of tumor genetics. In particular analysis of cfDNA may be useful to guide selection of an individualized therapeutic regimen without requiring percutaneous biopsy." @default.
