• W2917283081 abstract "To report long-term outcomes of radioembolization (Y90) of hepatocellular carcinoma (HCC) patients within Milan Criteria and study success rate of bridging to liver transplantation (LT) vs drop-out rate from LT waitlist due to HCC progression. With IRB approval, we studied HCC patients within Milan criteria treated with Y90 from 2004-2017. Listed patients for LT were studied for successful bridging to LT. Patients who dropped-out LT waitlist were further studied to determine drop-out rate due to HCC progression vs other causes. An intention-to-treat analysis was conducted. 319 HCC patients within Milan criteria had Y90. Child-Pugh class was: A: 164 (51%), B: 134 (42%) and C: 21 (7%), respectively. 185 (58%) were listed for LT, while 134 (42%) were not listed for various reasons: old age (n=47), substance abuse (n=12), cardiac comorbidities (n=18), other simultaneous cancer (n=12), non-compliance (n=8), distant residence (n=5), systemic comorbidities (n=8) and patient refusing LT (n=17). The majority of listed patients, 151/185 (82%), were successfully bridged to LT. 34 (18%) of the listed patients dropped-out; only 5 (3%) were due to HCC progression, while 12 (6%) refused LT, 3 (1.5%) developed other cancers, 8 (4%) had debilitating non-hepatic conditions, 2 (1%) were non-compliant, 3 (1.5%) moved out-of-state, and 1 (0.5%) died of variceal hemorrhage. Median overall survival (OS) of entire cohort was 93 (CI: 80-120) months. Median time between listing and LT was 7.7 (CI: 7-8.5) months. Median OS of 151 patients who had LT was 120 (CI: not reached) months. Median recurrence free survival post-LT was 120 (CI: 69-120) months, with only 13 recurrences. Non-transplanted (non-LT) patients were older than LT cohort (mean age: 70 vs. 60 years) (P<0.0001). Median OS of 168 non-LT patients was 37 (CI: 28-61) months. Non-LT CP A patients had OS 68 (CI: 45-80) months, CP B and CP C were 21 (CI: 14-30) and 6 (CI: 4-11) months, respectively. Y90 is effective in bridging HCC to LT with only 3% of patients dropped-out LT waitlist due to HCC progression over a 14-year period.
