• W2920802461 abstract "Purpose: A 34-year-old male with no past medical history presented to the emergency department with abdominal pain, nausea, and vomiting, all of which had been worsening over the past month. Consequently, he had decreased appetite and weight loss. The patient admitted to fevers, chills, and social alcohol use but denied illicit drug use. Physical examination was remarkable for epigastric and right upper quadrant tenderness to palpation and left foot drop. Blood work showed mild anemia, elevated amylase, lipase, alkaline phosphatase, and liver function tests. Computed tomography of the abdomen revealed an enlarged pancreas inseparable from the second portion of the duodenum with multiple hypodense masses throughout the pancreas (Figure 1). An upper endoscopy was performed and demonstrated a circumferential, ulcerated mass in the first and second portions of the duodenum (Figure 2). Biopsies and immunohistochemical analysis revealed a T-cell lymphoma. Subsequent PET scan for staging demonstrated tumor uptake in the pancreas, duodenum, and cerebellum. Neoplasms of the small bowel account for 0.4% of all cancers in the United States and represent 2% of all gastrointestinal malignancies. The gastrointestinal tract is the primary site of extra nodal non-Hodgkin's Lymphoma. While lymphomas account for 1-4% of GI malignancies, the majority are of B cell origin (66%). Most of these are located in the ileum due to the preponderance of lymphoid tissue in this region; only rarely will the duodenum be affected. Patients typically present with non-specific complaints such as abdominal pain, anorexia/weight loss, bowel obstruction, abdominal mass, diarrhea, and nausea/vomiting. When T cell lymphomas are diagnosed, they carry a very poor prognosis because they are usually identified in advanced stages.
