- W2922140481 abstract "Introduction: Assessment of liver stiffness using ultrasound elastography is a validated technique for determining liver fibrosis severity. Transient elastography (TE) [ Echosens] uses a mechanically induced impulse shear-wave to obtain a single measurement. Shear-Wave elastography (SWE) uses a dedicated ultrasound machine [SuperSonic Imagine], to generate a MACH cone shear-wave pulse to produce realtime 2D/ 3D images.. However, not all elastography exams are successful at generating a result. The purpose of this study was to determine what factors play a role in not being able to obtain a valid liver stiffness score by TE and SWE. Methods: Consecutive patients seen in our clinic over a 90 day period who underwent assessment of liver stiffnes by both TE and SWE were included. TE incorporated use of the M probe or XL probe and the IRQ/Med value had to be < 30%. The Supersonic Imagine used a XC6-1 probe and SWE measurements required a Stability Index of at least 90%. Factors examined for both TE and SWE included age, gender, BMI, Diabetes, HTN, Alcohol Use, Smoking, and Sleep Apnea. Statistical anlaysis used Fishers Exact test. Results: 100 patients (58F;42M} are included in the analysis. Mean age 55.3 yrs. Mean BMI was 30.8 and ranged from 16.5 up to 55.4. 52 pts had HTN, 23 DM, 29 drank alcohol, 36 smoked and 3 had sleep apnea. An unreliable liver stiffness score was noted in 6 pts with the TE probe and in 7 with SWE. All 6 pts in whom the TE failed, the SWE was successful, and vice versa. Failure to obtain TE liver stiffness score was associated with a BMI > 40 (p < 0.002) as well as presence of DM (p<0.03). Failure to obatin a SWE liver stiffness score was associated only with presence of DM (p<0.05) but not BMI or any of the other factors examined. Conclusion: Obtaining a valid liver sitiffness score was achieved in 100% of patients by incorporating the use of both ultrasonographic elastography modalities; TE and SWE. Despite incorporation of XL probe for TE device, a BMI of > 40 remains a risk factor for an unattainable result. However, BMI did not affect SWE measuremenrs. On univariate analysis, Diabetes did affect both TE and SWE success rate. Further studies are required to elucidate this fidning." @default.
