- W2922271438 abstract "Purpose: In 2008, the American College of Gastroenterology (ACG) modified their colorectal screening guidelines regarding the approach to patients with family histories of colorectal polyps. The changes made from the 2000 ACG recommendations highlighting the importance of obtaining detailed history from patients that includes the number, type and size of adenomas found in their relatives. This information then allows recommendations regarding screening regimens either as average-risk or high risk. This study aims to estimate the proportion of patients undergoing screening colonscopy for a family history of poylps that have knowledge of the advanced features found on colonoscopy; specifically the type, size and number, of their family member's polyps. Methods: The endoscopic database at our institution was queried for those patients undergoing colonoscopy for an indication of family history of polyps from January 2009-March 2010. 100 patient charts were reviewed and the following information was collected regarding their family history of polyps - which family member was diagnosed with polyps, age at diagnosis, type of polyp (pre-cancerous/adenomatous or non-adenomatous), size of polyp and number of polyps found. Age of family member and type of polyps were entered at the time of the procedure, when known; Patients whose data collection was incomplete by chart review were then contacted by phone to obtain information regarding their family history. Binomial 95% confidence intervals were calculated. Results: Of the 100 patient charts reviewed, complete data collection for 73 patients was obtained. The age ranged from 29-95 years with an average age of 52.2 years. Male patients comprised 23.2% of patient population and the remainder were female; among this group 82.2% were Caucasian, 15.1% were African American, 2.7% were of another ethnicity. When asked about their family history of polyps, 23.2% (95% confidence interval [CI], 14-35%) did not know which family member had polyps, 42.5% (95% CI: 31-55%) did not know the age at diagnosis, 71.2% (95% CI: 59-81%) did not know the family member's polyp type, 97.3% (95% CI: 90-100%) did not know the polyp size, and 90.4% (95% CI: 81-96%) did not know the number of polyps found. Conclusion: Patient knowledge regarding detail of family history of polyp data (i.e., age at diagnosis, number, size and histology) appears often unknown or incomplete. Thus, it does not appear feasible or practical to incorporate this level of information in colon cancer screening guidelines. It is likely that the most cautious level of screening/surveillance would be chosen as default for those with missing detail regarding family history of polyps, and this would contribute significantly to over-surveillance." @default.
