- W2922366808 abstract "Removal of objects endoscopically has been historically based a culmination of size, shape and symptomatology. Ingested objects more than 2 cm in diameter or 5 cm in length are recommended for removal as they lead to obstruction. As for sharp objects, they should also be removed promptly for risk of perforation. Our case report conflicts with the current guideline of observing cylindrical batteries in the gastrointestinal tract after ingestion and endoscopic removal. A 40 year old female with history of schizoaffective disorder, major depressive disorder, anxiety, asthma, history of prior ingestion of batteries in 2015, presented to our emergency room after ingesting 10 AA batteries one half hour before presenting to ED. The patient had reported hearing voices, telling her to eat batteries. She complained of acute epigastric pain after ingestion. She denied nausea, vomiting or diarrhea, hematemesis, hematochezia or melena and denied chewing the batteries. In the emergency room vitals were within normal range, no abnormal findings were noted on chemistry or complete blood cell panel. The acetaminophen, lithium, salicylate and blood alcohol levels were all undetectable. An X-ray of the chest and abdomen had showed multiple AA batteries contained within the stomach only. During EGD, 9 AA batteries were found in the dependent portion of the fundus and were successfully removed with snare. Multiple numerous thermal mucosal burns and mucosal erosions were detectable in the oropharynx, esophagus and fundus of stomach, minimally oozing. Repeat X-ray of chest and abdomen showed no remaining foreign bodies. The patient was admitted to IMCU for further management started on intravenous proton pump inhibitor (PPI) and oral sucralfate, and placed on arms length for suicidal ideation. The patient was observed without any complications and discharged on per os PPI and sucralfate to inpatient psychiatric care. When to remove foreign bodies from the stomach is a clinical uncertainty. Objects remaining in the stomach longer than 48 hours should be removed. As discussed in ASGE guidelines, cylindrical battery ingestions are less common, and data on the outcomes of these ingestions are limited. This reported clinical case demonstrates and highlights an important observation, in the event a patient consumes cylindrical batteries, they should be removed via endoscopy as soon as possible to prevent early mucosal damage and sequelae of battery ingestion.1887_A Figure 1.
