- W2923288889 abstract "By 2015, at the end of the Millennium Development Goals (MDGs) era, each year more than 6 million children younger than 5 years and 1 million adolescents were dying each year and more than 300 000 maternal deaths were occurring every year.1WHOGlobal health estimates 2016: disease burden by cause, age, sex, by country and by region, 2000–2016. World Health Organization, Geneva2018Google Scholar These deaths are a substantial health problem and social injustice because most cases are preventable and are concentrated in rural populations and poorer communities in sub-Saharan Africa and south Asia. Within the context of the Sustainable Development Goals (2016–30) the UN Secretary General launched the Global Strategy for Women's, Children's, and Adolescents' Health (henceforth referred to as the Global Strategy).2Temmerman M Khosla R Bhutta ZA Bustreo F Towards a new Global Strategy for Women's, Children's and Adolescents' Health.BMJ. 2015; 351: h4414Crossref PubMed Scopus (35) Google Scholar The Global Strategy was underpinned by lessons learnt from the MDGs and other new evidence, providing a roadmap for action based on country experiences.3Kuruvilla S Bustreo F Kuo T et al.The Global Strategy for Women's, Children's and Adolescents' Health (2016–2030): a roadmap based on evidence and country experience.Bull World Health Organ. 2016; 94: 398-400Crossref PubMed Scopus (137) Google Scholar The Global Strategy is ambitious to achieve nothing less than a transformation in health and sustainable development by 2030 for all women, children, and adolescents worldwide.2Temmerman M Khosla R Bhutta ZA Bustreo F Towards a new Global Strategy for Women's, Children's and Adolescents' Health.BMJ. 2015; 351: h4414Crossref PubMed Scopus (35) Google Scholar In a 2016 Assembly Resolution,4WHOCommitting to implementation of the Global Strategy for omen's, Children's and Adolescents' Health. World Health Organization, Geneva2016Google Scholar WHO Member States reaffirmed their commitment to the health and wellbeing of women, children, and adolescents. They requested the necessary technical support from the WHO Director-General for the national plans to end the high numbers of preventable deaths in this population and to report annually to the General Assembly. In the 2017 election speeches, the new Director-General provided a powerful platform for the highest attainable standard of health and wellbeing for all, with the health of women, children, and adolescents included among his top five priorities.5Ghebreyesus TA Curriculum vitae: Dr Tedros Adhanom Ghebreyesus.https://www.who.int/dg/election/cv-tedros-en.pdfDate accessed: March 15, 2019Google Scholar These commitments contributed to a growing optimism for ambitious action; however, we are concerned by some early indications that the promises are now under threat. First, in January, 2019, the substantial threat to global health of the high rates of morbidity and preventable mortality among women, children, and adolescents was not mentioned in the WHO's ten threats to tackle in 2019.6WHOTen health issues WHO will tackle this year. World Health Organization, Geneva2019Google Scholar Second, at the WHO Executive Board meeting in January, 2019, Member States discussed primary health-care resolution and a resolution associated with the UN high-level meeting on universal health coverage.7WHOStrengthening essential public health functions in support of the achievement of universal health coverage. World Health Organization, Geneva2016Google Scholar Neither resolution explicitly used the words women, children, or adolescents. Intense lobbying by many partners resulted in references being made to women and to sexual and reproductive health and rights in the final version; however, children and adolescents are not mentioned. Finally, the new WHO structure, unveiled just before International Women's Day, erased by a stroke of a pen the words women, children, and adolescents in the WHO's organigram. We applaud the WHO transformation for its progressive ideas, such as the creation of a digital health department and an innovation hub; however, we call upon WHO to reconsider the promise to women, children, and adolescents with dedicated senior leadership in the coordinating authority for health. Resources must be earmarked, and a holistic view is needed to address the health and development concerns of these groups effectively and efficiently. This approach includes putting women, children, and adolescents at the centre of primary health care and universal health coverage. We must build on the achievements of the past and fulfil the commitments made. Women, children, and adolescents are crucial to the overall success of the 2030 agenda. We declare no competing interests." @default.
