- W2923607108 abstract "The delivery of research in a complex, time critical and often stressful environment such as the Emergency Department (ED) poses many challenges. Many studies cross speciality boundaries and require close interdisciplinary collaboration. Explaining research studies to patients and relatives and obtaining consent in an emergency setting requires excellent communication skills. Simulation offers the opportunity to practise these skills in an environment safe for both the patient and the practitioner. Our ED has vast experience in simulation based education (SBE) and works with a cohort of simulated patients (SPs) to deliver training to a wide variety of health professionals. There is little in the literature on the use of SPs for the refinement of processes and communication in research. This abstract reviews our current use of SPs for this purpose, offers an early example and makes suggestions for further evaluation and development of this work. SPs are coached to act as potential participants or relatives in research studies. They are given guidance to simulate the various reactions and questions researchers will encounter. Scenarios are made as immersive as possible and performed in-situ. Feedback is given to the researchers on their verbal and non-verbal interactions. ProMISe was an ED/Critical Care trial of early goal-directed resuscitation for septic shock that required complex interventions, such as Central Venous Pressure (CVP) monitoring and the use of vasopressors. A simulation in the resuscitation room familiarised the research team with the equipment and recruitment process.It highlighted gaps in knowledge and potential problems such as equipment availability and accountability handovers. It also offered the opportunity to practise explaining a complicated study. This methodology has subsequently been used for many of our studies. The simulation of adverse events is another valuable learning opportunity for the research team. Feedback from the research team on the use of simulation, despite pre-conceived apprehension, has been promising.They have reported increased confidence in approaching patients and in dealing with difficult situations. Simulation is a valuable adjunct to study set up, particularly in the challenging environment of ED. It exposes latent problems in the environment and process, and increases the confidence of the research team. We would like to do more robust evaluation of the value of simulation in this context and explore other research options. An example would be linguistic analysis of the interactions between researchers and potential participants, with a view to identifying approaches that would optimise recruitment.
