- W2933735912 abstract "This chapter reviews long-anticipated optical mapping studies of atrial fibrillation (AF) in human atria that show rotational sources where targeted ablation can eliminate AF. It then discusses clinical mapping on both sides of the classical debate whether AF is driven by sources or is a self-sustaining disorder, highlighting the limitations of each. The chapter also focuses on clinical, animal, in vitro and in silico analyses that support localized sources. It describes methods for performing rotor mapping in patients, attempting to reconcile the largely favorable results of clinical AF source ablation with divergent studies. Several multicenter and single-center studies have shown that ablation of localized rotors and focal sources for human AF can improve success over conventional pulmonary vein isolation alone. Future mechanistic studies should compare maps of AF generated by any modality concurrently with other modalities, particularly optical mapping in human atria and delayed enhancement MRI atrial imaging." @default.
