U Ĺžen je orgĂĄnovĂĄ manifestace a zĂĄvaĹžnost poruchy modifikovĂĄna procesem X inaktivace. CĂlem prĂĄce bylo popsat prvnĂ pĹĂpad rodiny s Danonovou nemocĂ diagnostikovanĂ˝ v ÄeskĂŠ republice. DiagnĂłza byla stanovena u 27letĂŠho nemocnĂŠho muĹže s terminĂĄlnĂm srdeÄnĂm selhĂĄnĂm na podkladÄ hypertrofickĂŠ kardiomyopatie, nĂĄslednÄ pak u jeho sestry a synovce. Byla zaloĹžena na prĹŻkazu autofagickĂŠho procesu v biopsii kosternĂho svalu, absenci proteinu LAMP2 imunohistochemickĂ˝m vyĹĄetĹenĂm a prĹŻkazem patogennĂ mutace v pĹĂsluĹĄnĂŠm genu. KardiologickĂŠ nĂĄlezy byly kompatibilnĂ s diagnĂłzou hypertrofickĂŠ kardiomyopatie bez obstrukce vĂ˝tokovĂŠho traktu LK, v EKG obraze byly pĹĂtomny znĂĄmky preexcitace. Ke klinickĂŠ manifestaci onemocnÄnĂ doĹĄlo v deseti letech (u sestry ve 12 letech, u synovce v osmi letech). U probanda a jeho sestry byl dokumentovĂĄn rozvoj systolickĂŠ dysfunkce levĂŠ komory srdeÄnĂ a progrese do terminĂĄlnĂho srdeÄnĂho selhĂĄnĂ ve tĹetĂm decenniu. OnemocnÄnĂ u nich bylo ĂşspÄĹĄnÄ ĹeĹĄeno srdeÄnĂ transplantacĂ. PostiĹženĂ kosternĂho svalstva u nemocnĂ˝ch mÄlo odraz ve zvĂ˝ĹĄenĂŠ sĂŠrovĂŠ koncentraci kreatinkinĂĄzy." @default.
