- W2938477083 abstract "Purpose of this study was to generate experimental data for optimization of the hepatitis A epidemiological surveillance system. Materials and Methods. Filtration experiments were done using cultural hepatitis A virus HAS-15 strain. Viral suspension was filtered through various combinations of membranes to assess flow speed and viral adsorption properties. Concentration of the pathogen in sewer water samples (n=120) was carried out by adsorption method with long exposure time. Viral RNA was detected by real-time PCR. Anti-HAV prevalence rate was studied by ELISA method in the population of N. Novgorod 2007-2017 (n=2365, age: 1-60 years old). To determine the amount of genetic variability of the pathogen 76 viral isolates from N. Novgorod in 2000-2017 were genotyped using an optimized amplification of VP1/2A region technique. Results and discussion. Results of this study suggest usage of a combination of cardboard or polypropylene pre-filter and polyamide filter to increase filtration speed up to 5 times without sensitivity loss. Environmental monitoring for hepatitis A virus contamination is invaluable asset in order to prevent new cases of the infection. 13% samples of sewer water in N. Novgorod 2006-2018 were positive for viral RNA. Amount of positive samples in 2018 was 3 times higher than in 2017 (27,6% and 9,8% correspondingly, p<0,05). Increased virus occurrence in sewer water is a sign of active pathogen circulation and can be used as a predictor of high morbidity. Genotyping of the hepatitis A virus reveals that subtype IA dominated (97,4±3,67%), and IB was identified rarely (2,6±3,67%). Systematic immunological survey is the only way to reveal true hepatitis A prevalence due to frequent asymptomatic cases. Anti-HAV seroprevalence rate in indicator age groups of children 1-4 and 5-9 years old can be used as another predictor of high morbidity. Seroprevalence control level for these groups was calculated (17,2% and 18,2% correspondingly). In 2013 seroprevalence rate significantly increased in 1-4 year old group from 5,4±3,7% to 29,2±7,0%, and in 5-9 years old group from 6,6±2,8% to 25,6±4,8%, thus exceeding control levels. These data were used to predict the start of epidemic phase of hepatitis A in 2014. A survey in 2017 revealed that 23,14±3,8% of children bear anti-HAV in group 1-4 years old.
