- W2948414059 abstract "要旨 当院ではキーワード覚知方式によるドクターカーを運行しており，小児科専門医資格と救急科専門医資格を併せ持つ小児救急医が搭乗することにより，積極的に小児事案に運用している。我々は年間約100件の小児例への要請に対応してきたが，運行時間は平日の午前8時半から午後5時に限定しているため，全日時間の約20%の時間しか対応できていない。本調査では地域における小児救急疾患の実情からドクターカーの小児に対する当院での至適な運行時間を検討した。2014年の管轄消防におけるすべての小児搬送患者（1,363件）の救急搬送票から通報内容がドクターカー要請キーワードに合致する症例950件を対象に覚知時間・覚知内容・病着時の重症度を抽出した。その結果をもとに運行時間を変更することにより増加する症例見込み数と増加する人件費による収支予測を行った。1時間でも運行時間を延長すれば，増加する人件費により減収となることがわかった。よって運行時間を延長するのではなく，変更するために8時間の運行時間で最も症例数が多い時間帯を調査した。平日の午後0時から午後8時が最も対応症例数が多くなることがわかった。現行の当院での人員配置で，より現実的にドクターカーで対応する小児患者数を増やすために，運行時間を延長するのではなく，運行時間を平日の午後0時から午後8時までに変更することが最も至適であると考えた。" @default.
