- W2948774931 abstract "The efficacy of balloon enteroscopy-assisted endoscopic retrograde cholangiopancreatography (BE-ERCP) for patients with surgically altered gastrointestinal anatomy (Billroth II gastrectomy (B-2) or Roux-en-Y anastomosis (R-Y)) has been reported previously. However, although this method allows the endoscope to reach the papilla, successful biliary cannulation is difficult. To resolve these difficult situations, we developed a novel biliary cannulation method using a unique, uneven, double-lumen cannula (Uneven method). The UDLC (PIOLAX, Tokyo, Japan) has two lumens measuring 0.025 and 0.035 inches in diameter. The orifice of each lumen is uneven, thereby forming a channel at the tip of the UDLC. For BE-ERCP cases, UDLC is intubated to papilla through the pancreatic guidewire (P-GW) in distal lumen initially. It results to straighten the pancreatic duct and the common channel, and to stabilize the papilla. As a result, the GW from proximal lumen toward to bile duct direction with a certain probability and no effort to adjust the axis of the catheter to the bile duct direction is needed. Quick approach for biliary cannulation is possible. We confirmed the usefulness of this method for BE-ERCP." @default.
