• W2949207735 abstract "Objective: Chronic thromboembolic pulmonary hypertension (CTEPH) due to chronic pressure overload of right ventricle (RV) leads to heart failure with poor outcomes. For inoperable CHEPH balloon pulmonary angioplasty (BPA) is a treatment option with good long-term results. The 12-lead electrocardiogram (ECG) is available and unexpensive method to evaluate right ventricular hypertrophy (RVH). We evaluated the relationship between the improvement in hemodynamic and functional status and ECG in patients with CTEPH who underwent BPA treatment. Design and method: In 15 patients with inoperable CTEPH we examined the hemodynamic data before and 1 year after BPA. In addition, the sequential ECG findings for right ventricular hypertrophy (RVH) were assessed. Results: The mean pulmonary arterial pressure (mPAP) decreased from 48,6 ± 13,6 to 37,6 ± 11,8 mmHg, pulmonary vascular resistance (PVR) decreased from 10,6 ± 4,6 to 7 ± 2,6 WU, cardiac index (CI) increased from 2,2 ± 0,5 to 2,5 ± 0,8 L/min/m2 The changes in the mPAP (δmPAP) correlated with the changes in the amplitudes of the ECGs: δS wave in lead V1 (R = −0, 716, p 0,003), δR in lead avR (R = 0,535, p 0,040), δR wave in lead V5 (R = −0,514, p 0,05). Also correlation with δQRS width (R = −0,600, p 0,018) and δP waves in lead II (R = 0,611, p 0,016) were found. The changes in δNYHA functional class correlated with the δmean heart rate (R = −0,550, p 0,034) and δQRS width (R = −0,574, p 0,025). The changes in δCI correlated with the δP waves in lead II (R = 0,6575, p 0,025) and δS wave in lead V5 (R = 0,583, p 0,023). In 1 year follow-up the improvement in the amplitudes of S wave in lead V5 and R in V1 + S in V5 indicated a better functional status (increasing 6 minute walk distance (6MWD) and decreasing dyspnea). Conclusions: BPA therapy reduced the pulmonary arterial pressure and improved patients functional status that associated with an improvement in the ECG findings related to RVH (especially amplitudes of S wave in lead V5 and R in V1 + S in V5)." @default.
