- W2952806086 abstract "Objective: Resistant Hypertension (RHT) is associated with higher risk of Obstructive Sleep Apnea (OSA). OSA and aortic stiffness (AS) measured by pulse wave velocity (PWV) are independent risk factors for cardiovascular (CV) events. The continuous positive airway pressure (CPAP) is the choice treatment for moderate-severe OSA, with a questionable influence on blood pressure levels. However, there aren’t enough studies to evaluate whether CPAP is able to reduce CV risk by reducing AS. Our objective was evaluate prospectively, in a randomized clinical trial, PWV in resistant hypertensives with moderate and severe OSA, before and after 6 months CPAP use in comparison with the control group. Design and method: Patients from a large cohort of resistant hypertensives with moderate-severe OSA were randomized for a 6-month CPAP use or not. PWV was performed at the beginning and at the end of the period. The intergroup comparison of PWV changes were calculated using a linear model adjusted by the respective baseline PWV values. Subanalysis was performed in patients with CPAP's good adherence, initial uncontrolled 24h-ambulatory blood pressure monitoring (ABPM) and initial PWV > 10m / s. Results: A total of 99 patients (40% men with mean age of 60 ± 8 years) were randomized into 2 groups (CPAP = 48 and Control = 51). The treatment was maintained over 6 months. The 2 groups (CPAP and Control) had similar clinical characteristics, 24-h ABPM and PWV. There was a tendency to increase PWV before and after the study period in both groups, regardless of previous blood pressure control and CPAP adherence. In the subanalysis of those with increased initial PWV, a similar reduction was observed in the 2 groups, with no statistical significance.Conclusions: Six months CPAP use did not change the degree of AS measured by PWV in this cohort, even in patients with good adherence and in those with increased PWV before CPAP. Possibly requiring a longer time use for effective changes in endothelial dysfunction and in AS itself." @default.
