- W2955546871 abstract "A 72-year-old man presented with a 2-month history of rapidly enlarging, well-demarcated erythema with varicose veins of the chest wall (top). He had a past history of Hodgkin lymphoma of a left axillary lymph node 4 years previously, for which he had received local radiotherapy, achieving complete remission. He recalled similar skin erythema involving the left axilla at initial presentation. Physical examination did not show any lymphadenopathy. A skin biopsy of the erythematous area revealed extensive haemangiectasia throughout the dermis. A positron emission tomography–computed tomography scan demonstrated hypermetabolic masses with maximal standard uptake value of 20·24 in the soft tissue of chest wall (bottom). Scattered hypermetabolic signals were related to the sternum and in the mediastinum, left supraclavicular region and both axillae. A fine needle aspiration biopsy of the mass revealed Hodgkin lymphoma, leading to a diagnosis of cutaneous flushing associated with relapsed Hodgkin lymphoma. The patient received chemotherapy with doxorubicin, bleomycin, vinblastine and dacarbazine (ABVD regimen). The erythema on the chest gradually subsided along with remission of the Hodgkin lymphoma. The flushing phenomenon may represent a physiological or a pathological skin reaction. Neoplastic causes of cutaneous flushing are associated with the release of vasoactive mediators. Hodgkin lymphoma is a rare cause of this phenomenon. Localized cutaneous flushing should be considered as a possible warning sign for Hodgkin lymphoma, especially in patients with a previous relevant history. Thorough investigation is indicated if the flushing is persistent. This case highlights the importance of recognizing unusual presenting symptoms and signs of haematological malignancy." @default.
