• W2955572063 abstract "Usage of traditional/complementary medicines or supplements (TCM/S) are common among Asians. However, there is a lack of information with regards to TCM/S-associated acute kidney injury (AKI), especially characteristics, renal outcome, morbidity and mortality. The aims of this study are to understand the characteristics, immediate outcomes and long-term sequelae of TCM/S-associated AKI in a tertiary hospital. This single-centre, prospective cohort study was performed among patients who were admitted to Selayang Hospital, Malaysia from 1st March 2017 till 30th May 2017. AKI definition was based on KDIGO 2012 criteria that occurred in patients within 48 hours of hospitalization. All AKI patients were identified through the hospital laboratory information system. Patients with positive history of current TCM/S usage were recruited for this study. Demographic data, clinical data, renal outcomes and survival outcomes were collected for all patients admitted during the study period, and a year after the AKI episode. Chronic kidney disease (CKD) were defined based on the KDIGO 2012 CKD criteria. Estimated glomerular filtration rate (eGFR) was calculated via CKD-EPI equation. Information on renal and patient outcome for patients who lost to follow-up were obtained from phone follow-up interview and the National Renal Registry Database. Results were analyzed using SPSS Version 23. Of a total of 717 admissions with AKI, 37 admissions (5.16%) were associated with TCM/S use. There was a slight predominance of male gender at 20 cases (54.1%). The mean age of the TCM/S-associated AKI was 60.1±16.1 years. The Malay ethnicity predominated the cases slightly with 20 cases (54.1%). Six (16.2%) deaths occurred during their hospitalisation. At time of discharge, four (10.8%) required long-term dialysis, four (10.8%) had eGFR of 15-29 ml/min/1.73m2, four (10.8%) had eGFR of 30-44 ml/min/1.73m2 and five (13.5%) had eGFR of 45-59 ml/min/1.73m2. Eight (21.6%) had normal renal function upon discharge. At 1 year, of those who discharged well, five (16.1%) died, while one (2.8%) progressed to end-stage renal disease (ESRD). Seven (18.9%) subjects had decline in renal function, resulting in deterioration in CKD staging, while three (8.1%) subjects demonstrated improvement of renal function. The overall total one-year mortality in TCM/S-associated AKI cases was 29.7% (n=11), while 13.5% (n=5) of the subjects had ESRD within one year. Only six cases (16.2%) of TCM/S-associated AKI had normal renal function at 1 year. TCM/S-associated AKI cases contributed to 5.16% of the AKI cases in our centre. TCM/S-associated AKI subjects had poor outcome with overall mortality rate of 29.7% at one year, ESRD rate of 13.5% and only 16.2% had recovered with normal renal function at one year. Educational efforts should be reinforced among the Asian population to increase awareness of the potential risks of AKI associated with TCM/S use." @default.
