- W2958211977 abstract "Background and Aims: Regeneration of remnant liver in early post operative period determines outcome in donors in living donor liver transplantation (LDLT). The aim of the current study was to assess factors influencing liver regeneration after donor hepatectomy. Methods: Prospective observational study conducted including 153 consecutive donors who underwent donor hepatectomy for LDLT from January 2015 till December 2016. Total liver volume (TLV), estimated graft volume and remnant liver volume (RLV) was calculated preoperatively by contrast enhanced computed tomography (CECT) of abdomen. Absolute and Percentage growth of remnant liver in donor was estimated on POD 7 by CT abdomen. Demographic and clinical variables were analyzed as potential predictors of liver regeneration. Results: 118 donors underwent right hepatectomy excluding MHV (Middle Hepatic Vein) (RH), 29 underwent left hepatectomy including MHV (LH) and 6 donors had left lateral sectionectomy (LLS). Median percentage growth of remnant liver on POD 7 was 46.14 percent (51.74% and 35.32% and 17.38% for RH, LH and LLS respectively). On univariate analysis female donors (p = 0.051), right lobe graft (p = 0.001), normal liver parenchyma on USG (p = 0.042), lower TLV (p = 0.029), RLV (p=<0.001), remnant liver volume body weight ratio (RLVBWR) (p=<0.001), preoperative ALT (p = 0.017), AST (p = 0.035) and higher POD 7 ALP (p = 0.033), POD 7 GGT (p = 0.006) were found to be predictors of greater liver regeneration. Stepwise logistic regression analysis showed lower RLV (P = 0.008), RLVBWR (p = 0.011, in the absence of RLV) and preoperative ALT level (p = 0.021) were most significant factors predictive of regeneration of the remnant liver. Conclusions: Significant amount of liver regeneration occurs at the end of first post operative week. Donors with low RLV and RLVBWR had a tendency for faster regeneration. Lower preoperative ALT level as a marker quality of liver parenchyma was predictive of liver regeneration. The authors have none to declare." @default.
