- W2959366 abstract "This chapter discusses whether interventions into the germline might justify a restriction of reproductive autonomy to limit possible side effects to the treated individual. Although there is a disagreement about the assessment of human gene therapy in general, two major ethical landmarks are widely accepted; firstly, the aim of the intervention, distinguishing prevention or cure of a defined disease from enhancement, and secondly the range of the treatment. Another important fact when assessing the risk of germline modifications in somatic gene therapy is the developmental stage at the time of intervention. In utero therapies may involve a higher risk of germline modifications, which cannot sufficiently be quantified yet. Every efficient gene therapy performed during the first two weeks of individual development will equally affect both germline and soma cells. Every measure adopted for limiting individual reproductive autonomy must tend to protect the subsequent generations from harm by preventing defective gametes from being transmitted, and attempt to avoid early embryos being affected during pregnancy. Thus, before any gene-therapeutic intervention, gametes should be saved and stored, and contraception should be guaranteed as far as possible after the intervention. Gene therapy in human beings might carry severe risks for subsequent generations. The health of future offspring can be endangered by not only intended interventions in germline cells, but also by inadvertent effects of somatic gene therapy. Therefore, one should investigate whether interventions into the germline might justify a restriction of reproductive autonomy in order to limit possible side effects to the treated individual. In a first step, relevant germline interventions and their risk profile will be presented briefly. Subsequently, a definition of “reproductive autonomy” and the development of different methods for a gradual limitation of this fundamental right will enable us to discuss the legitimacy of such a restriction. By pointing out general ethical aspects and probable further developments, some considerations concerning the justification of restricting reproductive autonomy can be made. Some measures adopted so far will be illustrated, and a short summary of the actual discourse about the relevance of inadvertent germline effects will show the difficulties regarding risk assessment in this special case. It will then be suggested that, at this point, a risk-adjusted approach for restricting individual reproductive autonomy seems to be the best possible solution." @default.
