- W2961433720 abstract "Purpose: We report the unusual finding of melanosis of the ileum noted on screening colonoscopy. The patient is a 63-year -old West African woman, with a past medical history of hypertension, diabetes mellitus, stage 4 chronic renal insufficiency and secondary hyperparathyroidism, gout and chronic, non-iron deficiency anemia. Her home medications include: simvastatin, aspirin, iron sulfate 325 mg bid, metoprolol, spironolactone, losartan, insulin, nifedipine and sevelamer. Baseline labs include BUN/creatinine 104/3.1; Hb/Hct 10.3/30.7; MCV 91; Iron/TIBC/Transferrin sat% 87/292/30; Ferritin 1511. Endoscopy revealed a normal appearing colon; however, the ileum appeared speckled with dark punctate dots. Histology confirmed the clinical appearance of melanosis of the ileum, with hemosiderin deposits, particularly in the villi. Melanosis refers to the mucosal discoloration caused by pigment deposits in mucosal macrophages, commonly found in the colon, but which can occur anywhere in the gastrointestinal tract. Pigments described in GI melanosis include lipofuscinin, melanin, iron sulfide and hemosiderin. Melanosis of the ileum is very rarely observed but has been reported associated with chronic charcoal ingestion as well as in patients with renal disease receiving iron supplementation.
