- W2965424010 abstract "Recurrent implantation failure (RIF) is a heterogeneous clinical entity that affects a significant number of patients undergoing assisted reproductive technology treatment. For a proportion of these patients, the pathophysiological mechanism seems to be reduced endometrial receptivity. Ovarian stimulation for in vitro fertilization (IVF) has been shown to frequently result in abnormally elevated serum progesterone concentration on the day of triggering final oocyte maturation. This elevated progesterone has been convincingly shown to negatively affect endometrial receptivity and reduce the probability of pregnancy after a fresh embryo transfer. On the other hand, embryos from such cycles with elevated progesterone seem to have a similar probability of implantation when transferred in endometria that have not been expose to these high levels of progesterone, such as in frozen-thawed embryo transfer cycles or donor/ recipient cycles. There is some evidence suggesting that patients with recurrent implantation failures after IVF are more likely to exhibit elevated progesterone on the day of triggering final oocyte maturation. Furthermore, it has been also shown that some women have a propensity to exhibit higher progesterone concentration at the end of their ovarian stimulation regardless if the intensity of ovarian stimulation. These findings do indicate that elevated progesterone on the day of triggering final oocyte maturation might be playing a significant role in some patients with RIF. The management of such cases of RIF could be either prevention of progesterone elevation or segmentation of the cycle and embryo transfer in a subsequent frozen-thawed cycle in an unaffected endometrium. Although the latter approach appears to be the most popular, randomized data confirming the efficacy and safety of this approach is still lacking." @default.
