- W2968397921 abstract "The retrospective cohort study by Sine et al1 once again documents that severely functionally impaired nursing home residents are likely to experience nonhip (femoral shaft and tibia-fibula), non–fall-related lower extremity fractures. It has been almost three decades since these types of fractures, referred to as minimal trauma fractures (MTFs),2 spontaneous fractures,3 spontaneous long bone insufficiency fractures,4 and “transfer” and “turning” fractures,5 were first described in nursing home residents. Often mistakenly attributed by family members to physical abuse by aides, little research has been conducted to determine the mechanical and nonmechanical transfer and turning techniques and preventative therapies that minimize the risks for MTFs. Consequently, the training programs for certified nursing home assistants6 and personal care aides7 in New York State, for example, while requiring the demonstration of multiple transfers and turning procedures, do not specifically address how to minimize risks for and detect MTFs. Despite the identification of risk factors associated with MTFs, there is a need for further research into the biomechanical stresses of spontaneous and caregiver-assisted resident movements and lifting, turning, and transferring techniques to determine whether specific ones are associated with MTFs. Techniques that minimize the direct force and torque on long bones from care-related maneuvers at the knee, especially in those with bilateral severe knee contractures,8, 9 deserve further study.
