- W2970842667 abstract "Background Adult Congenital Heart Disease (ACHD) is a highly specialised field of cardiology. Regardless of complexity, all ACHD patients should be seen at least once by an expert from a specialist centre and a care plan provided. The holistic care of ACHD patients is delivered by a multidisciplinary team with specialised knowledge. Although echocardiographic imaging is fundamental in the care of ACHD patients, these patients require access to many other services. Certain conditions will always require continuing specialist care, including (but not exclusively): patients with congenitally corrected transposition of the great arteries, repaired tetralogy of Fallot, repaired coarctation of the aorta, and patients who have undergone atrial switch, arterial switch, or Fontan procedures/operations. Unfortunately, ACHD patients do get lost to specialist follow up, often in the transition from childhood to adulthood. The loss to appropriate cardiology follow-up can be substantial and this ‘gap’ in care may put patients at increased risk of adverse outcome. These patients may present to a local medical centre or hospital leading to referrals to adult echocardiography service providers. The adult cardiac sonographer without specialist knowledge should not feel obliged to perform an echocardiogram (echo) on an individual with ACHD, due to the complexities often associated. In particular, when not performed by those with expertise in CHD, diagnoses can be missed. However, there will be circumstances when it is not appropriate to decline an echo. In such instances it is best practice to apply a systematic segmental approach to the echo to adequately demonstrate anatomy and any corrective surgery/procedures. It is important that the sonographer is aware of their knowledge gaps as well as the limitations of the test. Wherever possible, patients should be referred to a specialist centre to ensure an adequate continuum of care. Presentation Outline The purpose of these presentations: ▪ Illustrate why the care of adults with CHD should have input from an ACHD Specialist. ▪ Demonstrate the importance of understanding the ACHD patient's condition to ensure an adequate echo assessment. ▪ Provide practical echocardiographic imaging techniques in the assessment of ACHD including: ▪ A comprehensive systematic segmental approach. ▪ Echocardiographic examples demonstrating this systematic segmental approach. ▪ Case Studies ▪ Highlight the following: ▪ Importance of following a systematic segmental approach and describing only what you see. ▪Never feel obliged to perform an echocardiogram on a patient with CHD when unfamiliar with the patient's condition. ▪ Always consider referring the patient to an ACHD Specialist Centre." @default.
