- W2972618847 abstract "Introduction: The application of crowdsourcing in healthcare allows studies to garner large sample sizes while decreasing costs and saving time. Ratings from the general public provide assessments of how children with craniofacial differences are viewed. A survey on the attractiveness of children with single suture craniosynostosis yielded lower ratings in comparison to their normal counterparts. This rating did not improve with post-surgical intervention (Collett et al., 2013): highlighting the divide between public perception and objective surgical outcomes. A question that remains unanswered is at what level of surgical correction does the public perceive a head shape to be “normal”? The purpose of this study was to determine the average “acceptable” head shape of sagittal craniosynostosis perceived by laypersons. Methods: This study utilized a crowdsourcing method. A 3D image of a patient with severe sagittal craniosynostosis (based on cephalic index (CI)) was used as the index case representing extreme scaphocephaly deformity. An age and gender matched normative infant skull was the desired result for surgical correction. Using a step-wise wrap/unwrap method, 11 still images of lateral and top down views were generated with 10% incremental changes from the extreme abnormal to normal head shape. Each photo was presented twice to measure inter-rater reliability. An online survey was designed in which participants were recruited via the Sickkids Twitter channel and invited to indicate their perception of the head shape as “normal” or “abnormal”. Consensus was reached when 70% of respondents considered the head shape as “normal”. Inter-rater realibility was measured using Kappa score. Results: There were 821 completed surveys. Females consistuted 68% of the participants. From a top view, the public considered the head shape as “normal” when 80% of the sagittal deformity has been corrected. Furthermore, from a side view, the public reported the head shape to be normal when 70% of the sagittal deformity has been corrected. The inter-rater realiability measured by Kappa score was 0.89. Conclusion: This study showed that using crowdsourcing technique the public considers a 70% correction in extreme sagittal craniosynostosis to be of a normal and acceptable head shape.
