- W2973131964 abstract "To investigate the changes in bone mineral density (BMD) in patients with gynecological cancer treated with intensity-modulated radiation therapy (IMRT). Ninety-one patients with cervical or endometrial cancers treated by cisplatin(DDP) based concurrent chemo-radiotherapy(CRT) or IMRT alone from September 2017 to July 2018. We collected 44 patients’ BMD records of pre- irradiation and post-irradiation after 3 months. We compared menopause status, Body Mass Index (BMI), radical surgery or not, the concurrent chemotherapy characteristics of the patients with changes of T scores of lumbar vertebra 1 (L1), L2 and femur respectively. The significance of differences was assessed by using the paired samples t test, and Welch’s unpaired t test. The median age of the patients was 53 years (range 31-78). Thirty-seven patients (84%) were diagnosed as cervical cancer, seven patients (16%) were endometrial cancer. Thirty-one received surgery and thirteen not. The patients were staged according to the FIGO staging system: 12 cases were stage I, 16 were stage II, and 16 were stage III. For the entire group, the mean BMDs of femur before IMRT compared with those after 3 months IMRT were -0.77 and -0.95 (p=0.001). In the premenopausal patients, the mean difference of femur BMDs decreased more significantly compared with those in postmenopausal patients (-0.27 vs -0.04, p=0.017). The L1 and L2 of sixteen patients (36.7%) who received RT to the para-aortic lymph node region were in the irradiated region. In these cases, the mean difference of L1, L2 BMDs decreased more significantly compared with those who were out the irradiated region (-0.24 vs 0.06, p=0.035; -0.26 vs 0.12, p=0.036), while the difference of femur change between the two group were not significantly. In CRT group, the mean difference BMDs of L1 decreased more significantly compared with patients treated with only RT (-0.19 vs 0.14, p=0.011). A decrease in BMD in the irradiated region after RT was observed after 3 months. In addition, the menopause status, concurrent chemotherapy, and RT may influence the location and the extent of decrease." @default.
