It remains unclear whether these delays influence outcomes. This review aimed to assess the impact of delays to RC on survival by systematic review and meta-analysis using currently available data. Methods Medline (PubMed) and Ovid Gateway (Embase and Ovid) were searched using protocol-driven search terms. The identified studies were then assessed according to inclusion and exclusion criteria. Meta-analyses were conducted based on type of delay (BC diagnosis to RC; transurethral resection (TURBT) to RC). Random-effects models were used whereby the presence of a delay (as defined by each study) was the exposure variable and overall survival was the outcome of interest, for which pooled HRs were calculated. Results Seventeen studies were eligible for inclusion, of which six were subsequently included in the meta-analysis. The studies varied greatly in how delays were defined. When studying total delay between BC diagnosis and RC, the meta-analysis revealed a pooled HR for overall death of 1.32 (95% CI: 1.13-1.54) when comparing those with a longer delay to those with a shorter delay (as defined by each study). For delay between TURBT and RC, we found a pooled HR of 1.17 (95% CI: 0.89-1.52) for death from all causes. The main limitation in the meta-analyses was study heterogeneity. Conclusions This review highlights the importance of treating those patients requiring RC without delay. Several factors impact delay, such as other medical conditions and scheduling, and these must be monitored by medical practitioners in order to minimise unnecessary delays to improve survival.
