- W2977263556 abstract "Purpose: Food hypersensitivity (intolerance to specific foods) (FHS) is commonly reported in clinical settings. However, double-blind, placebo-controlled food challenges often fail to confirm an intolerance and the majority of individuals who report adverse reaction to specific foods do not react so when they receive the food unknowingly. Currently, the mechanisms underlying these symptoms have not been revealed limiting available treatment options. Purpose: To investigate the role of psychological factors in the pathogenesis of FSH and food-related gastrointestinal symptoms. Methods: Study population included subjects with chronic recurrent symptoms of FHS and healthy controls (HC). All subjects were recruited from the general population by advertising. Subjects were evaluated using validated questionnaires. Variables of interest include I. Demographic and Clinical Parameters II. Food items sensitivities assessing reported intolerance to 90 common food items in American diet. III. Food sensitivity symptoms including GI and non-GI food related symptoms IV. Psychosocial factors assessing emotion, coping and stress using validated questionnaires (Brief Symptom Inventory (BSI), Visceral Sensitivity Index (VSI), Perceived Stress Scale, Coping Strategies Questionnaire, Eysenck Personality Inventory, Highly Sensitive Person Questionnaire (HSP), Multidimensional Health Locus of Control Scales, and Illness Attitude Scale). Results: A total of 44 subjects, 20 FSH and 24 HC, mean age 32 years, 75% females were investigated. Compared to HC, subjects with FHS scored significantly higher on multiple psychological variables including VSI, Perceived Stress, HSP and neuroticism (p<0.01 for all variables by t-test). The number of reported food items sensitivities, frequency of days with food-related symptoms and symptoms severity significantly correlated with psychological variables (by Pearson correlation; Table 1). The groups differences in number of food sensitivities, frequency and severity of food-related symptoms became insignificant when controlling for psychological factors (ANCOVA analyses) suggesting that these differences in food related symptoms can be explained by psychological variables.Table: No Caption available.Conclusion: We provide first evidence for the significant effect of psychological factors on the clinical manifestations of FHS. Patient with FHS have significantly higher psychological difficulties. These psychological difficulties strongly correlate with food related symptoms. Evaluating and addressing psychological factors may help the treatment of patients with FHS." @default.
