- W2977554914 abstract "Introduction: Hepatitis C virus (HCV) infection is a now considered a treatable disease with Direct-Acting Antiviral Agents (DAAs). In reported clinical trials, treatments with DAAs have shown Sustained Viral Response (SVR) rates of 92-97%. Among the US Veteran population, treatment outcomes in the past with protease inhibitors such as boceprevir and telapravir the SVR rates were substantially lower than the those reported in randomized controlled trials 50% vs 66-79%. The objective of our study was to determine the SVR rates in Veterans with HCV who have completed treatment with DAAs in a real world sample. Methods: We performed a retrospective chart review of HCV infected Veterans at the VA Central California Health Care System (VACCHCS) who had completed treatment for their HCV with DAAs and had their HCV RNA checked 12 weeks after completion of their treatment (SVR12) from 11/1/2013 to 1/31/2016. Data was stratified based on age, race, cirrhosis, HCV genotype, prior treatment, DAA regimen and SVR results. Statistical analysis using STATA version 11.1 was performed. Groups were compared using C-square/Fischer's Exact Test. Results: 130 Veterans treated with DAAs at the VACCHCS met our inclusion criteria. Overall our SVR response rate was 84% compared to clinical study rates of 92-97%. Mean age among the Veterans was 62 +/- 6 years, 98% were men, 58% were Caucasians, 20% Hispanic-American and 11% were African-Americans. 66% of our study population had cirrhosis and 47% were treatment experienced. SVR rates were statistically different based on genotypes with 89% SVR for genotype 1a, 87% for 1b, 71% for genotype 2, 63% for genotype 3a. Among the treatment failures, 53% were genotype 1, 14% were genotype 2 and 29% were genotype 3. 81% of the treatment failures had cirrhosis. The predominant regimen among genotype 1 failures was Ledispravir/Sofosbuvir without ribavirin. Conclusion: Response rates to DAAs among our real world sample of Veterans with chronic HCV were lower than clinical trials. However, our study population was older, predominantly cirrhotic and treatment experienced.
