- W2977927255 abstract "Introduction: Fecal microbiota transplantation (FMT) is an established therapy for RCDI and is effective across all age groups. Studies suggest that the risk of recurrent Clostridium difficile infection (RCDI) is several folds higher in patients >65 years of age as compared to younger patients. However, the effect of aging on fecal microbiota has not been examined in RCDI patients. Methods: Pre and post-FMT fecal samples from 11 RCDI patients were analyzed. Group G65 comprised of older patients [>65 years (n=5; 4F, 1M; mean age=73.8 years)] and Group L65 included younger patients [ < 65 years (n=6; 4F, 2M; mean age=50 years)]. Total metagenomic DNA was used for 16S rRNA gene-specific PCR amplification and pooled amplicon pyrosequencing. Microbiota compositions were analyzed (CloVR-16S) and differences in fecal microbial diversity at the phylum, family and genus levels of the two groups were then examined. Results: The overall alpha diversity of the gut microbiome was much higher in the older patients than younger patients before they got their FMT, however, it did not reach statistical significance (Figure 1). Microbial diversity increased after FMT in both groups, but it was significantly pronounced in the younger patients (p < 0.05).Figure 1At the phylum level, Firmicutes were more abundant in older patients when compared to the younger patients (pre-FMT). Although concentration of proteobacteria decreased in both groups post-FMT, it was only statistically significant in the older group of patients (Figure 2). At the family level, abundance of Turicibacteraceae, Lachnospiraceae and Bifidobacteriaceae increased significantly post-FMT in older patients while the abundance of Erysipelotrichaceae and Coriobacteriaceae increased significantly post-FMT in younger patients (Figure 3). At the genus level, Coprococcus and Bifidobacterium became more abundant in older patients after FMT. The Fermicutes/Bacteroides ratio was similar in both the groups before and after FMT.Figure 2Figure 3Conclusion: Older patients failed to have significant rise in microbial diversity following FMT as compared to younger patients. Marked increase in Bifidobacteriaceae and Lachnospiraceae was noted in the elderly. Changes in these forms of bacteria, which are associated with immunomodulation may be responsible for clinical improvement in elderly patients." @default.
