- W2977929742 abstract "Purpose: In recent years, there has been a substantial increase in the incidence of microscopic colitis. The reasons for this increased incidence includes increased clinical awareness, more frequent performance of colon biopsies, and increased use of medications that cause microscopic colitis. Microscopic colitis is identified on colonoscopy with biopsy in 10-20% patients with non-bloody diarrhea with a higher yield in the elderly. The role of detecting microscopic colitis in asymptomatic patients can be significant, especially if this would predispose them to symptoms of diarrhea in the future. Grass Valley, CA, a small-sized town in the foothills of Sierra Nevada, has a high elderly population, making it a suitable setting for this study. The aim of this study was to assess the yield of random colon biopsies performed in asymptomatic patients with normal appearing mucosa on visual inspection during screening and surveillance colonoscopies. Methods: This was a prospective study from Sierra Nevada Gastroenterology Medical Associates, Inc., a community-based private practice gastroenterology group in Grass Valley, CA that performed outpatient colonoscopies at Sierra Endoscopy Center, a free-standing ambulatory surgery center. Only patients with normal bowel habits undergoing screening and surveillance colonoscopies and with normal appearing mucosa on visual inspection were included in the study. Informed consent was obtained with IRB approval. Data was collected on age, gender, race, past medical history and medication use. Four random biopsies, each from ascending/transverse colon and descending/sigmoid colon were obtained for routine histology. The pathologist was blinded to patients clinical information. Results: A total of 103 patients, 50 males and 53 females, with mean age 64 yrs were included in the study. 99% of the patients were Caucasian. See Table below for results.Table: Results. Demographics, clinical and histology findingsConclusion: There was no yield of colon biopsies in asymptomatic patients in detecting microscopic colitis. Non-specific colitis of unknown clinical significance was detected in some patients. Bowel prep, infection, and nonspecific foreign body granulomas appeared to be the likely etiology. It is interesting to note that there was a higher yield of colon biopsies from the right colon compared to left colon, favoring the use of colonoscopy for evaluation of mucosal disease." @default.
