- W2977935061 abstract "Introduction: Current guidelines for the timing of endoscopy for foreign body swallowers (FBS) recommend intervention within 12-24 hours. The primary aim of our study was to examine the effect of time intervals from both ingestion and hospitalization to endoscopy on the risk of complications among FBS. Methods: Data was prospectively collected, including patient encounters from March 2015 to March 2016 on FBS at a large city hospital. Statistical analysis was performed using Wilcoxon rank sum test (quantitative variables) and Fisher exact test (categorical variables), and significance was confirmed with multivariate logistic regression analysis. Results: Our study included 57 patient encounters (93% Male, mean age 29). 58% of the patients were African American, 21% Hispanic, 14% Caucasian and 7% other. All ingestions were intentional, 86% of the patients were incarcerated and 79% had a psychiatric history (of whom 91% were taking psychiatric medication). 95% of the patient encounters had previously ingested a foreign body. 98% of the foreign bodies were successfully retrieved. 39% of the ingested objects were blunt (i.e.battery or urinal cap), 40% sharp (i.e. plastic knife or nail cutter) and 21% both (more than one object ingested). Sharp objects were ingested in 65% of the encounters with mucosal injury. Mean object length was 5.3cm. There were no major reported complications or perforations in our study. Mucosal injury was reported in 45% of the cases (97% superficial, 3% deep). Among the patients who suffered mucosal injury, the mean time from hospitalization to endoscopy (HTE) was 18.6 hours (SD+/-11.63) and the median time was 16.9 hours (Interquartile Range 10.9-20.6;p=0.04); mean time from ingestion to endoscopy (ITE) was 39.9 hours (SD+/-28.09) and the median time was 30 hours (IQR 24-45.5;p=0.22). The overall range of HTE was 1.0-114.0 hours for all patients. Among the patients who did not suffer from mucosal injury, the mean HTE time was 31.3 hours (SD +/-24.15) and the median time was 24.2 hours (IQR 14.5-45.0;p=0.04); the mean ITE time was 29.8 hours (SD+/-78.55) and the median time was 43.0 hours (IQR 27-59.5;p=0.22). Conclusion: In our prospective study, we did not observe a significant impact of time between ingestion or hospitalization and endoscopy on serious complication rates among FBS. Longer HTE was associated with lower incidence of mucosal injury, suggesting that patients with higher incidence of injury had ingested a more noxious object warranting more urgent retrieval." @default.
