- W2977959718 abstract "Purpose: Bowel wall thickening (BWT) is frequently observed by abdominal computed tomography imaging (CT scan). Many diseases have been reported to cause BWT, i.e. carcinoma, inflammatory bowel disease, ischemic colitis and edematous wall. However, it has also been purported to represent incidental, clinically insignificant finding. This clinical dilemma is challenging when the clinical indications for follow-up endoscopy are not obvious. The aim of our study is to identify the predictors of abnormal endoscopic and clinically significant pathological findings of biopsy performed for the bowel wall thickening reported on CT scans of HIV infected patients. Methods: This is a retrospective review of 38 subjects who underwent endoscopy and biopsy for further evaluation of BWT found on CT scan. They were either of the chest alone, or the abdomen and pelvis, or both. All procedures were performed between January 1, 2008 and June 30, 2010 for gastrointestinal symptoms. The chart review was conducted by Internal Medicine residents and Gastroenterology fellows at St. Michael's Medical Center, an inner-city community hospital in Newark, New Jersey, USA. Results: Twenty six (68.4%) out of 38 subjects had abnormal endoscopic findings. Of those, 15 (39.4%) subjects had clinically significant pathological findings on endoscopic biopsies of corresponding BWT site found on CT scan. An Albumin level of less than 3.5 is associated with abnormal endoscopic findings (p = 0.043). Hypoalbuminemia is an independent predictor of clinically significant abnormal histology found on endoscopic biopsies (p=0.005). The hemoglobin level is a predictor of abnormal endoscopic findings (p=0.034). Conclusion: The results of our study demonstrated that the presence of anemia and hypoalbuminemia in HIV patients with BWT found on CT scan should be further evaluated by means of endoscopy and biopsy. This may reduce the delay in performing the procedure and facilitate more successful outcomes." @default.
