- W2977963460 abstract "Introduction: Recent evidence suggests that the incidence of colorectal cancer(CRC) is increasing in young adults(YAs) aged ≤40 years. This observation may warrant earlier colonoscopy CRC screening recommendations. In order to evaluate bowel preparation experience in younger patients, we performed a post hoc analysis to investigate the safety, efficacy, and tolerability of a low-volume bowel preparation in YAs. Methods: Data were obtained from 2 large(n=1201) phase 3, randomized, multicenter, assessorblinded studies that investigated split-dose and day-before administration of a low-volume preparation containing sodium picosulfate, magnesium oxide, and anhydrous citric acid(P/MC) compared with per labeled dosing of 2L polyethylene glycol solution and two 5-mg bisacodyl tablets(PEG+bis) in adults preparing for colonoscopy. Data were analyzed separately for YAs. Colon cleansing was rated using a modified Aronchick scale. Patient tolerability was ascertained from a 7-item questionnaire administered the day of colonoscopy prior to sedation. Safety was assessed by physical exam, serial hematology and chemistries, 5 individual electrocardiograms, and the incidence of adverse events(AEs). Results: A total of 37 YA patients in the intent-to-treat population were randomized to the P/MC treatment groups and 34 to PEG+bis. A significantly greater proportion of YAs in the P/MC treatment groups rated their preparation as “easy”/“very easy” to consume compared with PEG+bis(splitdose, 91%vs18%,P < 0.0001; day-before, 93%vs18%,P < 0.0001). Preparation taste was rated as “excellent”/“good” in a significantly higher proportion of P/MC patients in both studies(split-dose, 61%vs6%,P=0.0065; day-before, 79%vs6%,P < 0.0001). AEs reported in P/MC patients included headache(8%), nausea(8%), and vomiting(5%). AEs in PEG+bis patients included nausea(11%), vomiting(14%), and no headache. Endoscopists rated colon cleansing quality as “excellent”/“good” in a numerically greater proportion of patients in the P/MC groups compared with the PEG+bis groups(split-dose, 96%vs88%; day-before, 93%vs88%); however, these differences did not achieve statistical significance. Conclusion: In YA patients, low-volume P/MC provided quality bowel cleansing and a significantly better patient experience compared with 2L PEG+bis. Further study is needed to determine if the improved tolerability of P/MC will translate into improved screening guideline adherence." @default.
