- W2977983352 abstract "Introduction: The purpose of this project is to examine how well Gastroenterologists utilize DEXA scan for osteoporosis screening among IBD patients on long-term steroid in a tertiary academic medical center. Methods: In the year 2011, all patients in our tertiary academic health system who carry a diagnosis of either Crohn's disease or Ulcerative colitis, and were on steroid therapy were selected. Long term steroid use was defined as cumulative usage of systemic steroid for more than three months. 186 patients were originally included. After chart review, patients who did not have diagnosis of UC or CD, and patients who did not meet the criteria for long term steroid use were excluded. Chart review covered the year of 2011 up to 10/2014. Demographics, patient characteristics and DEXA scan reports were recorded. Results: 169 patients are included in the analysis. 77 had ulcerative colitis; 92 had Crohn's disease. 81 were female; 88 were male. 1) 168 patients are considered to be on long term prednisone. 1 patient was on only budesonide. 131 patients were on multiple courses of prednisone (78%), 31 patients were on long term lower dose prednisone (18%); 6 patients were on one course of prednisone with long taper (4%). 2) Of the 168 patients on long term prednisone, 78 had DEXA scan (46%); 59 (44%) of the 131 patients who were on multiple courses of prednisone had DEXA (34% of all the studied IBD patients), while 19 (61%) of the 31 patients who are long term low dose prednisone had DEXA (11% of all the studied IBD patients). None of the 6 patients who had long taper had DEXA. In all, 54% of all the studied IBD patients did not have a DEXA scan. 3) of the 78 patients who had DEXA scan, 33 (42%) (accounts for 19% of all the studied IBD patients) were diagnosed with osteoporosis, including 22 (37%) of 59 patients on multiple courses of prednisone (13% of all the studied IBD patients), and 11 (58%) of 19 patients on long term low dose prednisone (6% of all the studied IBD patients). Conclusion: Among IBD patients on long-term steroids, only less than half of patients had DEXA scan done for osteoporosis screening. There is high prevalence of osteoporosis in this patient population. A more standardized care model might help close the gaps and improve quality of care.Figure 1" @default.
