- W2977997680 abstract "Introduction: The clinical significance of sucrase deficiency (SD) in children and adults with gastrointestinal (GI) symptoms has not been well-characterized. SD has only recently been recognized to be more prevalent in the general population due to compound heterozygosity. This retrospective study assessed the clinical characteristics of pediatric patients found to have SD from small intestinal biopsies assayed for disaccharidases in a large cohort of patients during EGD. Methods: Endoscopic records of patients undergoing EGD over a 5-year period (2010-2015) at a freestanding endoscopy center serving 13 pediatric gastroenterologists were reviewed. The records of patients with SD were further analyzed to characterize their clinical characteristics and treatment outcomes. Results: Among 5,362 patients undergoing EGD, disaccharidase testing was performed in 963 (18%) at physician discretion. Of these, 73 (7.6%) were SD ( < 25 μmol/min/gm). Most had SD in combination with lactase deficiency (LD) (n=25; 34%) or pan-disaccharidase (n=44; 60%) deficiency with only four (5%) having isolated SD; age was 12.0±4.6 years and 45 (61.6%) were male. Most had abdominal pain (49; 78%) and/or diarrhea (27; 43%) although constipation, nausea, vomiting, weight loss/FTT, flatulence and bloating were not uncommon. Abnormal biopsies were found in 33 patients (45%) who were significantly younger (10.8±4.0 vs 13.0 ±4.9 years), had lower BMI percentile (46 ±32 vs. 63 ±34) with more episodes of weekly diarrhea (20 vs. 13) (for each, p < 0.05).Primary diagnoses following endoscopy were disaccharidase deficiency (28%), celiac disease (22%), duodenitis/ulcer (10%), IBD (10%), esophagitis/EoE (10%), gastropathies/ulcers (12%) and other diagnoses (21%). Among the 62 patients for whom follow-up information was available, 17 of 22 (72%) responded to diet changes alone; 10 of 16 (62%) responded to medications; and 14 of 19 (74%) responded to both diet changes and medication. In the diet group, 4 of 5 (80%) improved on diet changes and sucrase supplement; 11 of 16 (69%) responded to lactase restriction; and 1 of 2 (50%) responded when both were addressed; 18 of 23 (78%) responded to gluten-free or elimination diets. Conclusion: SD appears to be more prevalent than previously suspected and often occurs in tandem with LD. The full clinical picture, optimal diagnostic method and appropriate treatment algorithm for children and adults with SD need to be established by further research." @default.
