- W2978059611 abstract "Introduction: Smoker's melanosis is the brown to blackish discoloration of the digestive tract mucosa in oropharynx and laryngopharynx. Although it usually presents with multiple lesions, solitary pigmented lesion of melanocytic origin are very rare in the aero digestive tract which carries the same malignant potential. We present a case of middle aged woman with history of chronic smoking who presented for endoscopy and noted to have incidental isolated pigmented lesion in laryngopharynx. Case presentation: 59 years old African American presented to our institution for Esophagogastroduodenoscopy for evaluation of chronic dyspepsia. Past medical history was significant for morbid obesity, hypertension, hyperlipidemia, diabetes Mellitus, obstructive sleep apnea, asthma, and HIV controlled on antiretroviral medications. Patient gave 30 pack years smoking history. Physical examination and pre-endoscopic labs were unremarkable. Esophagogastroduodenoscopy revealed blackish-brown discoloration of the mucosa in the hypopharynx extending to the vallecular and small 5 mm healing ulcer seen in the pre-pyloric region [Figure 1]. Considering its location and unknown malignant potential of the lesion, we decided to refer the patient to ENT specialist to evaluate need of biopsy to rule out premalignant changes. ENT specialist later recommended close clinical observation with strict smoking cessation.Figure 1Discussion: Smoker's melanosis usually occurs in upto 30% of chronic smokers and gradually disappears after cessation of smoking. Isolated lesion in laryngopharynx is rare, but it is challenging to determine malignant potential of the pigmented lesions. Current dilemma is whether to pursue for tissue diagnosis or close clinical observation with endoscopic surveillance. Clinical studies with large numbers needed to determine the high risk endoscopic features of such lesions. We believe that careful endoscopic pharyngeal examination and prompt referral are essential to diagnose and prevent aero-digestive malignancy in chronic smokers." @default.
