- W2978351405 abstract "Introduction: Our aim was to examine the feasibility and safety of a novel endoscopic method for resection of gastric astric submucosal tumors (SMTs). Methods: Three consecutive patients, with gastric SMTs were enrolled in this study. During the procedure, a submucosal tunnel was endoscopically created starting approximately 4 cm proximal to the lesion. After careful submucosal dissection with carbon dioxide insufflation, the SMTs were completely removed, and the entrance of the tunnel closed using endoclips. Results: The procedure was performed successfully in all cases. The en bloc resection rate was 100%. Of the 3 gastric SMTs, 2 were located in the antrum, and one in the lower body. Two of the SMTs originated from the muscularis propria, 1 from the muscularis mucosae. The average lesion size was 1.6 cm (range 1.3 to 2.0 cm). The mean required time for the procedure was 52.7 minutes (range 49 to 55 minutes). No patient presented with bleeding, perforation, dysphagia or other complications after the operation. Histopathological examination showed lipoma in one patients and GI stromal tumor in two patients. All the patient kept asymptomatic during follow-up (mean 12.7 months, range 6 to 20 months). Conclusion: Our preliminary study demonstrated the feasibility and safety of endoscopic submucosal tunnel dissection (ESTD) in the treatment of gastric antrum SMTs.Figure 1: Case illustration of endoscopic submucosal tunnel dissection procedure. a) A SMT located at the greater curvature side of the low body. b) EUS revealed a 24.7×9.8 mm hypoechoic submucosal lesion. c) A 2 cm longitudinal mucosal incision was made, approximately 4 cm proximal to SMT. d) Submucosal dissection was done, creating a submucosal tunnel until the tumor was visible. e) Dissection was done along the margin of the tumor. f) Endoscopic view of submucosal tunnel after removal of tumor. g) Endoclips were used to close the entry of the submucosal tunnel. h) The resected specimen was a 1.5 cm lipoma." @default.
