- W2978387959 abstract "Purpose: High-resolution, high-definition colonoscopes (HDC) provide brighter and sharper images with a wider field of view than standard definition colonoscopes (SDC). This innovation is theoretically supposed to increase the endoscopists ability of detecting clinically significant lesions in the colon thus improving the polyp detection rate (PDR) and adenoma detection rate (ADR). However majority of the current data are from large academic institutions and/or have heterogeneous patient groups that have included high risk patients. In order to evaluate the impact of this technological innovation in a setting that is relevant to most clinical practices in the United States we compared the PDR and ADR in a cohort of average risk patients undergoing screening colonoscopies during a time period before (2010) and after (2011-5/2012) institution of HDC at a free standing endoscopy centre in a community setting. Methods: Endoscopic findings from 928 patients who underwent SDC were compared to 1466 patients who underwent HDC by 7 different endoscopists for routine cancer screening. With the assumption of a 30% polyp detection rate, the study sample was adequate to determine a 10% increase in polyp detection with 90% power at a significance level of 0.05. Known co-variates that impact PDR and ADR were statistically accounted for in the analysis. Results: The adenoma detection and polyp detection rate in the SDC group was 53.5% and 32.6% respectively which was not statistically different from the rates in the HDC group which were 50.4% and 30% respectively. Commonly known confounders that impact ADR and PDR such as withdrawal time, age, gender and ethnicity were similar between the two groups and were not found to impact the study endpoints (Table).Table: [1889] Characteristics and polyp and adenoma detection rates of patients undergoing SDC and HDCConclusion: After accounting for known confounders, we were unable to detect a clinically or statistically significant difference in PDR and ADR with the use of HD compared to SDC in a screening population.
