- W2978392313 abstract "To evaluate the efficacy of methotrexate to replace salpingectomy in the management of tubal ectopic pregnancies in Early Pregnancy Units (EPUs). Symptomatic women (pain, bleeding) with positive pregnancy test have been treated in the newly established EPU in Greece last year. Diagnosis of ectopic pregnancy was based on hCG levels alteration in the measurements calculated at 48 h apart as well as ultrasonic findings. Treatment s decision was made upon first clinical status of the patient and second availability of experienced ultrasonographer who could accurately explore ultrasonic features. Data of 132 ectopic pregnancies were evaluated retrospectively. 47 women underwent salpingectomy via laparoscopy due to internal bleeding (35.6%). Six patients refused treatment. Five women underwent D&C, followed by dropped levels of hCG. Wait and see methodology was applied on 23 patients. MTX was administered to 51 women; 50mg/m2 MTX was administered intramuscularly in 35 patients; four of them needed a second dose of MTX while 2 women were operated due to sustained increased levels of hCG. US guided intrasac administration of MTX (50mgr/m2) was applied in 16 women under general anesthesia. Six to 8 hours after MTX administration (im or intrasac), women were discharged. hCG levels were measured on outpatient basis the 4th, the 7th day after MTX treatment; thereafter weekly measurements were obtained until hCG levels reach zero. None of the 45 women needed further intervention. Five of the above MTX mentioned group became pregnant spontaneously (pregnancies ongoing). More than half of the recruited patients (57.5%) were successfully managed avoiding surgery. It seems that MTX administration, intramuscularly or intrasac, upon experience of the clinicians, as well as “wait and see” methodology could replace safely salpingectomy due to ectopic tubal pregnancies in young women, preserving efficiently their reproductive ability.
