- W2978907052 abstract "Introduction: Efficacy of colonoscopy is limited if the preparation regimen is unable to adequately clear the colon. Failure of proper cleansing most often reflects an inability of patients to properly follow the directions; however there may be other factors involved. Few studies to date have evaluated the patient's experience and barriers to preparation tolerability in an underserved group. We aimed to identify patient barriers and their effect on colon preparation tolerability in a multi-ethnic population. Methods: A prospective study using data obtained from a multi-language survey was performed. Demographic, medical, socioeconomic, and tolerability data were collected. Descriptive statistics were performed and multivariate logistic regression was employed to determine barriers to bowel prep tolerability. Results: 384 patients completed the survey. They were predominantly Hispanic (49%), not fluent in English (51%), had Medicaid(34.1%) or no insurance(27.3%), had a high school education(43.4%) and made less than $10,000 yearly(29.8%). Predominant health factors were diabetes (22.9%) and hypertension (44%). 16.8% of patients felt their difficulty in tolerating the prep was due to their current health issues. 30.3% of the patients found the prep intolerable while 11.6% found it “very intolerable”. Predominant reported side effects were bad taste in mouth (26.5%), lack of sleep (26.5%), stomach fullness (22.3%) and nausea/vomiting (16.8%). Predominant social barriers were time off work (22.6%), responsibilities at home (23.9%) and availability of appropriate food and drinks (23.2%). Female patients found the preparation less tolerable compared to male patients (OR .539, 95% CI .325-.894) and felt that their difficulty tolerating the preparation was due to their current health issues (OR 3.57, 95%CI 1.8-6.96). Use of an interpreter, insurance, marital status and education did not significantly affect the tolerability of the preparation. Conclusion: This study highlights the patient's perspective of bowel preparation tolerability and the barriers that contribute in a high-risk, vulnerable patient population. Although bowel preparation may be adequate for screening, nearly 1/3 of our patients found the method of preparation intolerable. This has potential to be an issue for a screening test, as most patients will need a repeat colonoscopy again in their lifetime. If they perceive the preparation as intolerable, they may not return to have a potentially life-saving procedure.Figure 1" @default.
