- W2978939771 abstract "Introduction: Colorectal cancer (CRC) remains the nation's third leading cause of mortality from cancer. Appropriate screening has shown to decrease the incidence and mortality of CRC. Current guidelines recommend screening all adults aged 50 to 75 with average risk. Screening rates however have remained particularly low in areas of poverty and among minorities. Our goal is to determine the CRC screening rate in an inner-city community health clinic and identify areas of improvement related to colon cancer screening in this underserved population. Methods: A search of our electronic medical records (E-Clinical Works) was done to identify all adult patients aged 50 to 55 years who were seen at our outpatient facility during the 2015 calendar year. We then determined whether CRC screening [colonoscopy, sigmoidoscopy, fecal occult blood test (FOBT) or fecal immunochemical test (FIT)] was ever performed, and if so, what specific findings were documented. Patients were excluded if colonoscopy was performed solely for diagnostic purposes. Results: Of the 556 patients in the target age-range, 20 patients were excluded for receiving diagnostic colonoscopies for hematochezia, persistent diarrhea/constipation or anemia. After the exclusion, 198 (37%) patients were screened for colon cancer. Of the patients screened, 154 (78%) received colonoscopies with 53% having documented polyps, and 44 (22%) received FIT/FOBT testing. Among the FIT/FOBT group, 2 (5%) had a positive test and subsequently underwent colonoscopy. Of note, 62 (18%) patients out of the unscreened 338 had tests ordered and did not undergo CRC screening. Conclusion: This study is the first quality improvement project undertaken at our facility addressing CRC screening. Our current screening rate of 37% is significantly below the citywide average of 70%. Several areas of improvement have been identified and implemented in our facility. Simple practices include introducing culturally and linguistically tailored campaigns and educational material, telephone outreach, and espousing the concept of ‘any screening test is better than none.’ Given that 18% of unscreened patients did not complete the appropriately ordered tests means more outreach is needed. It is imperative to have individualized discussions to improve patient literacy and extinguish any stigma or fear of CRC screening. This data shows that more interventions need to be done to ensure proper screening for a preventable disease." @default.
